Not sure where to start, or just wanting to have some of the paperwork taken care of?  You can download our Pre-Need Packet to begin preparations.  This packet includes our Death Certificate Worksheet, which is the information your Funeral Director will need to complete a death certificate and includes a Funeral Options form to better specify your wishes, and our Obituary Form to help with the writing of an obituary.  It also includes information about when a death occurs and the steps that take place within the first 24-hours.

You can also print our Cremation Authorization, which is the next of kin's permission for cremation.
